To ensure your candidate is fully and fairly considered, please write your nomination thoughtfully and thoroughly. The committee does not interview nominees, so it is important that your nomination is well-written and includes sufficient information to assist the committee.
Specific Examples Are Required and Necessary
Under each section of the form, you must provide specific examples that address how the nominee satisfies each criterion below. The quality of your nomination is directly linked to the strength of these examples.
Please be sure to answer every question completely and provide examples where appropriate. You must also include contact information for the three references you provide. Please inform the references that you are including their information.
